книжка простенькая, для пациентов и их родственников, но не для врачей. Однако, выкладываю как образец общения врача с пациентом.

Между прочим, насколько я знаю, в России нет таких книжек, где специалист простым доходчивым языком рассказывает пациенту о его болезни, о правилах жизни, о том, что следует делать и каким специалистам обращаться,.. ну что-то такого курса молодого бойца

Василий, а чего бы вам с коллегами не издать такую книжку:

"Эпилепсия: 1001 ответ доктора Василия"

Вот там и обо всем написать: и о имитаторах эпилепсии и о важности ВЭЭГ, нейрохирургии, ВНС и многом многом другом - просто и доходчиво. ИМХО пациентам это нужно.

Насчет возможности очага в стволовых струкутрах у человека.

Вот ищу не абстракты а нормальные статьи, такие знаете, такие бронебойные.

Обязательно найду! Хотя пригляделся, действительно очень спорный вопрос в литературе.

Brainstem triggers absence seizures in human generalized epilepsy

Simultaneous analysis of brainstem auditory evoked potentials (BAEPs) with reference to electroencephalography (EEG) was designed to examine the brainstem function corresponding to the EEG event. With this method, we investigated the brainstem function pre- and during the paroxysmal discharge in human absence seizures classified as primary generalized epilepsy (PGE). Two types of functional change in the lower brainstem were revealed as parameters of wave-III components (amplitude and area) of BAEPs without significant change in the upper brainstem. One was long-range biphasic fluctuation (acceleration followed by abrupt deceleration with the maximum -6.4+/-3.2 s before the seizure onset), and the other was rhythmic oscillation with 3 Hz. The latter, synchronized with the cortical spike-and-wave complex, imposed on the descending slope of the former. One important point is that both preceded the onset of cortical paroxysmal discharge. The results reappraise the classical hypothesis of "centrencephalic system" on seizure generating mechanism in human PGE. The results prove the primary triggering role of the lower brainstem that is independent of sleep-related synchronizations. The method is applicable to other types of EEG event for the investigation of brainstem involvement. Copyright 1999 Elsevier Science B.V.

Да, хорошо, потом я в отдельную тему все сложу..
Кстати работ по вызванным потенциалам и эпилепсии немало.
Но мы то обсуждали именно генерацию эпиактивности в стволе. Всетаки эпиактивность отличается от стволовых билатеральных вспышек. Хотя синхронизирующее влияние срединных структур в эпилептогенезе сложно отрицать.

Вчера наша секция на Человеке и Лекарство прошла по ВЭЭГ в диагностике...
Уровень понравился, зал был полный, все заинтересованные, видно, что в теме...
Очень понравилось, отзывы у докторов постпрашивал, вроде все довольны..
На следующий год будем зал больше просить... Опять народ в проходах стоял. И это без всякой рекламы...

Уровень понравился, зал был полный, все заинтересованные, видно, что в теме...
Очень понравилось, отзывы у докторов постпрашивал, вроде все довольны..
На следующий год будем зал больше просить... Опять народ в проходах стоял. И это без всякой рекламы...

Василий, какие именно срединные структуры имеете ввиду? А что за механизмы синхронизации? Пардон за простые вопросы. Можно было б книжку взять, но ... так ведь быстрее

Вот выводы:

4.3. Conclusion

On the basis of our study, it is assumed that nature of seizures originating in the MRF consists of varied patterns in the following order (from mild to severe symptoms): hyperactivity, a running/circling/bouncing seizure, tonic seizure, a tonic-clonic seizure, and amygdala kindling-like seizures with EEG seizure discharge involving forebrain structures. The MRF can be kindled, and subsequent AM kindling utilizes not only the strengthened vertical (limbic-brainstem) connection but also the proconvulsant neuroplastic changes that have been already established by MRF kindling.
